The Limitations of Insurance
When insurance companies cover part of your healthcare costs, they gain significant influence over the services you can receive. My partner, an oncologist, spends a substantial amount of time negotiating with insurance company doctors to approve potentially life-extending treatments. This scenario isn’t exclusive to critical treatments; it extends to routine services like STI testing. Insurance companies, by necessity, have to decline some services to remain viable, impacting the frequency and comprehensiveness of care.

The Complexity of Insurance
Insurance regulations vary widely across different states, leading to inconsistent and often confusing patient experiences. Navigating this maze can be frustrating, and you might find that your coverage doesn’t meet your needs as expected. I’m sure we’ve all received an unexpected bill in the mail after our insurance company only partially paid a laboratory testing invoice.

The Reality of “Free Clinics”
Community STI testing centers are also subject to rationing decisions, even if their services appear “free” to patients. These centers operate on grants or taxpayer funding. For instance, a local STI center might have a $4,000,000 grant to cover all expenses for a year. If this grant focuses on addressing a high syphilis risk in the community, the center may limit tests for other conditions like trichomoniasis to manage costs and serve more people. Essentially, the more comprehensive the testing panel, the fewer people the clinic can accommodate.

Some clinics also restrict access based on income. When I inquired about STI test costs at our local clinic, the first question was, “What is your income?”

A Look at the Canadian Perspective

Despite Canada’s nationalized healthcare system, services and drug costs are not unlimited or always free. Rationing is a reality there too, similar to the challenges faced by insurance companies and local charitable clinics.

It’s akin to being a teenager asking your parents for money—the inevitable questions are “What are you planning on buying?” and “Do you really need that?” Once you’re an adult, you make your own purchasing decisions because it’s your money.

Healthcare operates similarly. Whether it’s the government (Medicaid), a major insurance company, or a local STI clinic, the payer influences the services you receive. If maintaining non-monogamous sexual practices is important to you, paying for your sexual medical care out-of-pocket might be the only way to ensure you get the services you need.
